I am looking for a simple contact form service that will allow me to create contact forms with custom fields.

I've used mycontactform.com before, but I'm not crazy about being locked into eternal yearly fees. I've tried jotform.com, but their service doesn't seem very reliable.

Ideally I'd like to find a service that has a one-time fee. Does anyone have a any favorites they'd like to recommend? Thanks in advance for any help you can provide!
